Systemic autoimmune rheumatic diseases encompass a wide range of different inflammatory joint, tissue and vascular diseases, such as rheumatoid arthritis, systemic lupus erythematosus and systemic sclerosis. They are diagnosed on the clinical history, imaging techniques, and serological tests. Antibody detection is a useful diagnostic tool and can provide essential information for differential diagnostics. However, it is not always possible to detect autoantibodies, as not all target antigens have been discovered yet and detection systems are not yet available for every autoantibody.

Autoantibody testing is becoming increasingly important in rheumatological diagnostics. It delivers crucial information for differential diagnostics and sometimes even allows prediction of the disease progression or helps in the choice of therapy. Before diagnostic use, all serological tests undergo thorough investigation. They are only approved when they meet the respective criteria. However, a result obtained from these serological tests should never be the sole basis of a diagnosis as discrepancies may occur due to the very nature of autoimmunity.

Diagnostics of the different autoimmune-related rheumatic diseases pose a particular challenge to rheumatologists: They are confronted with at least 400 inflammatory joint, connective tissue or vascular diseases, which include subgroups sometimes requiring different treatments, and cause a great variety of symptoms. In addition to symptoms, specific antibodies are characterising features. But while there is great number of different antibodies, the prevalence is very low for some of them, making detection very challenging. However, for differential diagnostics in particular, autoantibody detection can give crucial hints and thus enable a fast therapy start for patients.
